What happens to Sam in season 13 episode 21?
After the boys take out a few vampires, they're ambushed, and while Dean is pinned by one, Sam is taken down by two, and while Dean watches, one of the vampires takes an enormous bite out of Sam's neck. Later, when Edgar visits a half-vampire Alan, seeking his help in hunting down the Alpha Vampire, it is revealed that Edgar had in fact killed Sam during the post-credits scene of the previous film (confirming that Sam had become a full fledged vampire, and not a half-vampire).

Sam's supernatural abilities are the result of him being fed Azazel's demon blood while he was an infant. Sam exhibits signs of precognition throughout the first season, manifesting as dreams of others' deaths and later as visions.
“You're digging at something, the intrepid detective,” he told Dean. “I want you to keep digging…it's about the souls.” With that, Sammy got his soul back, with a warning, “Don't scratch the wall because trust me, you're not going to like what happens.” Then Sam's screams pierced my soul. Ugh.

Castiel managed to pull Sam's body out by the last scene of season 5 — though his soul would remain in hell for 180 years (time is different down there) — and Lucifer would escape a few seasons later. And now, it's time for the other two to do the same.
In the next few episodes, he is seen with a cast on his right arm. This is because Jared Padalecki broke his wrist during a stunt on the previous episode, so his injury was written into the script to explain the presence of his cast in the following episodes.
